Initial Comments:
Based upon the findings of an unannounced onsite Medicare recertification survey conducted April 12, 2019, Fresenius Medical Care City Line Dialysis was identified to have the following standard level deficiencies that were determined to be in substantial compliance with the following requirements of 42 CFR, Part 494.62, Subpart B, Conditions for Coverage for End-Stage Renal Disease (ESRD) Facilities-Emergency Preparedness.

An onsite follow-up survey completed on April 12, 2019 found that Fresenius Medical Care City Line Dialysis had corrected the following condition cited under 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The condition was cited as a result of a Medicare recertification survey that was completed on March 11, 2019.
As a result of the follow-up survey, Fresenius Medical Care City Line Dialysis was identified to have the following standard level deficiencies that were determined to be in substantial compliance.
